Financial Arrangements
How are financial arrangements handled?
The facility charge incorporates all care during your procedure but does NOT include pre-procedure tests, anesthesia professional fee, pathology studies or special supplies.
All co-payments, deductibles and co-insurance are due at the time of admission as determined by your insurance benefits pertaining to ambulatory surgery centers. Our Admitting Office will attempt to contact you prior to your procedure as soon as benefits have been verified with your insurance carrier and we can estimate your co-insurance and/or deductible. Cosmetic procedures and self-pay accounts require full payment (100%) at the time of admission in order to be eligible for our prompt pay/self pay discount. If special payment arrangements are necessary, please call the Center's Admitting Office prior to the day of surgery. Our Admitting Office can be reached Monday through Friday between the hours of 8:00 AM and 5:00 PM at (904) 389-1077.
Our Central Business Office can be reached at (904) 737-9818 Monday through Friday from 8:00 am to 5:00 pm for any billing questions after your procedure and after the insurance claim has been filed.
Cash, Checks, Money Orders, MasterCard, Visa, American Express and Discover are accepted forms of payment.
