BRING INSURANCE CARDS, AND PICTURE I.D. (such as driver's license) when you come to the Surgery Center.

FOR PEDIATRIC PATIENTS, please make sure you have their social security number.

IF YOU HAVE A LIVING WILL we would like a copy for our records if possible.

IF YOU HAVE POWER OF ATTORNEY OR GUARDIANSHIP of the patient, please bring legal documentation of such.

THERE WILL BE NUMEROUS FORMS to sign and to be filled out that pertain to your state of health and personal information.

TO PROTECT YOUR PRIVACY the only people that will be allowed in the treatment area are the ones you list on the consent form. Only two people will be allowed in the treatment area at a time.
